PGY1 Residency Graduation Checklist
The PGY1 resident will complete the following items to satisfy graduation requirements.
GENERAL
Graduation and completion from an accredited school of pharmacy prior to residency training
Acquire Board of Pharmacy licensure within 3 months of beginning residency training
Maintain pharmacist license in good standing in the state of the primary practice site throughout the residency year
Attend all required scheduled residency functions, including staffing and patient care opportunities in the pharmacy, university events, meetings, monthly residency calls, required training, and other residency-related events requiring travel outside of assigned market.
Achieve a rating of Satisfactory Progress or better on 50% of the learning objectives identified for evaluation on the first two quarters of summative evaluation and on at least 75% of the learning objectives identified for evaluation by the end of quarter three
Perform a minimum of three lectures for professional audiences
Perform a minimum of three presentations for patient audiences
Document a minimum of 30 patient interventions in the 15 medication-related problem categories
Achieve a rating of Achieved on at least 75% of the learning objectives by the end of the residency
Complete a residency research project and present results in the form of a poster presentation, podium presentation, and manuscript suitable for journal submission
IMMUNIZATIONS
Administer IM vaccinations (e.g. Hep A, Hep B, meningococcal, HPV, influenza, pneumonia, shingles)
Administer 50 flu vaccines
Administer 8 Shingrix vaccines
Administer 8 pneumococcal vaccines where applicable
Administer 4 tetanus vaccines (Td or Tdap) where applicable
POINT OF CARE TESTING
Perform 10 A1C tests where applicable
Perform 20 blood glucose test where applicable
Perform 50 blood pressure and pulse readings
Perform 10 lipid panels
Request labs from other HCPs prior to individual patient appointments
WELLNESS PHARMACIST
Present 1 community program event
Conduct 1 memory screenings at pharmacy sites/community events (Mini-Cog™)
Participate in 2 off-site immunization clinics
Document 20 referrals to other HCPs using appropriate medium
Share 4 examples of referring clinic patients to other HCPs or services
Perform 6 Patient Snapshot Reflections (template)
WORKFLOW
Document 12 OTC recommendations and follow up (if possible) each quarter (48 total)
Observe 5 preceptors patient care while documentation while in workflow
Document 10 referrals and outcomes in EPS
Identify, dispense and counsel at least one patient on naloxone
Access OutcomesMTM® and EQUIPP™ platforms weekly
Conduct 3 months of MCPB audit of prescriptions from previous week
Conduct 3 monthly compliance audit
Participate in on-site compliance audit with pharmacy auditor
